Appointment Preparation

Complete and submit the online Child History and new patient forms. It is imperative that these be completed in advance of the initial appointment so that the therapist is prepared for the assessment or treatment.

Bring insurance cards. Submit online or bring medical records/reports, IFSP, IEP, ETR and/or other pertinent information.

Please arrive 15 minutes early to your first appointment to complete paperwork.

Wear tennis shoes and clothes that allow therapist to easily see bones, joints and muscles.

Bring braces/splints, glasses, hearing aids, and other adaptive equipment.

Bring extra bottles and/or snacks and diapers/supplies.

The Referral Process

A physician referral is required for physical therapy evaluation and treatment. Please have your physician fax the prescription to 330-867-2245.

You are not required to have a referral for behavior, occupational, reading, or speech therapy.

Before you begin, be sure to look at available insurance options.

We are excited to have you as a patient at LLA Therapy. Before we are able to service your needs, you must complete our required medical forms. After your initial intake call, our schedulers will send you all the necessary forms for your appointment via email.
